Anita Forman

Anita L. Forman, 72 of Rapid City, died Dec. 9, 2008 at Munson Medical Center. A gathering will be held Friday, Dec. 12, at 4 p.m. at the Central Lake Chapel of Mortensen Funeral Homes, where the family will receive friends from 2 to 4 p.m. In Lieu of flowers, memorials may be directed to the Elk Rapids Schools Auditorium Fund, 707 E. 3rd St., Elk Rapids, MI 49629, where a memorial fund has been established in her name.
